FormChime

Terms of Service

Acceptance

By installing or using FormChime you agree to these Terms.

The service

FormChime sends Google Forms responses to LINE through your own LINE Official Account. You are responsible for your LINE account, its message quota and any LINE charges, and for complying with LINE’s and Google’s terms.

Plans and billing

Free: 2 forms, 50 notifications per month. Paid: US$3.99/month or US$39.99/year (up to 20 forms, up to 10 recipients), sold by Polar as Merchant of Record. Subscriptions renew automatically until cancelled; cancel anytime and access continues to the end of the paid period.

Pro purchases are not currently offered to customers in Japan.

Refunds

You can request a full refund within 14 days of your first payment by emailing [email protected]; refunds are issued through Polar. After that, you can cancel anytime, but renewal periods and partial periods are not refunded. This does not limit any mandatory consumer rights.

Acceptable use

Do not use the add-on for spam, unlawful content, or to message people who have not agreed to receive messages. Broadcast (paid plans, off by default) sends to all friends of your Official Account: do not broadcast responses that contain personal data.

No warranty

The service is provided “as is”. Delivery depends on Google and LINE and may be delayed or fail. FormChime is not an emergency alert system; do not rely on it for emergencies.

Limitation of liability

To the extent permitted by law, our total liability is limited to the fees you paid in the 12 months before the claim, except for damage caused by our intent or gross negligence.

Changes and termination

We may update these Terms or the service with notice on this page. Material changes (including price changes) will be announced at least 30 days in advance; if you do not agree, you may cancel before they take effect. You may stop using the service at any time.

Governing law

These Terms are governed by the laws of Taiwan (Republic of China), and the Taiwan Taipei District Court has jurisdiction as the court of first instance, without prejudice to mandatory consumer-protection laws of your place of residence.
